Prior to admission

Please contact our Admissions office on 3240 1213.

Prior to entering the hospital, you will be required to check in using the QLD Health App.

The Check In Qld app is compatible with the Apple iPhone, iPad and iPad touch operating iOS 10.0 or later, as well as Android devices using Android 5+. 

To download the app:

visit www.covid19.qld.gov.au/check-in-qld for links to download the app from Google Play or Apple App stores 
enter your details once when you first open the app 
select ‘Check in Now’ and scan or enter our code 

If you don’t have a Smartphone, our team will be able to assist you on arrival to the hospital.  

App security:

 
The Check In Qld app is safe, secure and private.
Using this digital check in means your details will be stored securely by the Queensland Government for 56 days to assist with COVID-19 contact tracing (if needed). 

On the day of admission

Please proceed to Main Reception, located on the Ground Floor, where you will be asked to sign some paperwork and pay any out-of-pocket expenses. 

Please bring the following with you: 

  • Medicare number
  • private health fund details, including membership number and policy details, if applicable
  • pension/concession card details, including DVA details, if applicable
  • pharmacy benefits card/safety net number, if applicable
  • general practitioner’s details
  • next of kin/emergency contact details
  • details of your regular medications (including vitamins and over-the-counter medicines), if applicable
  • X-rays or other tests related to this admission
  • credit cards - we accept most major credit cards, EFTPOS and bank cheques.  We do not accept personal or company cheques, AMEX or Diners Club
