Discharge information

Before you leave

To ensure you have the correct services or equipment to support you when you leave hospital, our nurses will start to plan for your discharge at the time of your admission. Our staff are available to work with you to plan your discharge and arrange community support if required. If you have any concerns regarding your discharge, please let the staff know as soon as possible.


Discharge time

Expected discharge time: 10:00am

Your doctor and nursing staff will let you know when you are ready to be discharged. If you are unable to be collected by 10:00am, you may be asked to move to an alternate location.

Please pre-arrange for someone to take you home and care for you on discharge.

If applicable, please remember to take with you:

  • X-rays, scans and ultrasound films
  • Details of your next appointment
  • Any medication which was brought into hospital, as well as any new treatment
  • A letter for community services
  • A nursing discharge summary

Self discharge

You may discharge yourself from hospital or from treatment at any time. However, you do so at your own risk and the hospital will ask you to sign a self-discharge form.
